Our Humanize MOST Cohort created a "Mesanized" Getting Started Survey available to everyone for import. This survey can be customized to meet the needs of your students in your class. There's a question in there about "how many asynchronous online courses have you taken," so you'll want to remove/revise that question if your course is not asynchronous online. Tweak the questions however you want to align them more with your course, your personality, your students, your discipline, and your teaching philosophy. Remember the goal of this is to actually use the data that students provide to better support them towards their success in the class. So that's they key!

Instructions

1. Use the link above to import the Getting Started Survey into your course.
2. Customize that Getting Started Survey to meet the needs of your course (add/remove/revise questions). 
3. Make that Survey available to your students to complete during the first week of your class.
